This program prepares the student with the knowledge and skills necessary for the expanding role of the nursing assistant in a long term care setting. On completion, the graduate will be prepared for employment as a nursing assistant in Long-term care, Assisted Living facilities, Sub-Acute Care, under the supervision of the RN/LPN. Clinical experience is provided at local long-term care facilities.

Evenings on the Daytona Campus
Admissions is the first step, on any Daytona State Campus. Once you have completed the necessary form, you may proceed to the Assessment Center for testing (you must have a photo ID). After you have seen an advisor ensuring you have met the TABE test requirements (see below), you must print the CNA Course Packet which is available below. The information will tell you what you must have completed by the first day of class. Read this packet carefully and completely and follow all directions, including a physical exam, Level II background check and drug screening. If any of the categories regarding criminal offenses are cause for concern, please contact Maria Smith at 386-506-3069 or email@example.com or call the CNA Registry at 1-850-245-4567. Order your CNA Lab Kit using the order form in the Course Packet. This is the only way you are to obtain your CNA Lab Kit which you must have the first week of class. Please take note of the order end date or you will pay late fees.
TABE test scores: 9th grade Math, 10th grade Reading and Language. If you have had college Algebra and English, you must bring your unofficial transcript to the Advising Department on your local campus. If you have specific scores on the CPT, SAT, or ACT you are exempted from this test, see an academic advisor for information. If your scores are low on the TABE, the advisor will have you enroll in Adult Education courses to work on your skill levels that are low. There you will receive assistance in passing the TABE. Once you are successful, your advisor will have you register for the next available CNA course.
Applicants who started high school in the 9th grade in 2003 or after at a Florida Public high school AND graduated from a Florida Public High School with a standard high school diploma or active duty military are not required to take the TABE placement test. If you have any questions about your exemption status, please contact an academic advisor.
Nursing Assistant (Long Term Care) Program Guide
Mandatory Orientation required: Wednesday, January 3, 2018 at 4:00 pm on the Daytona campus in building 320 room 234. Orientation will last approximately two hours.
